The Delaware Community Healthcare Access Program (CHAP) provides low cost or no cost primary care "medical homes" to individuals who are ineligible for the Diamond State Health Plan or the Delaware Healthy Children Program, yet within established income limits. Enrollment in CHAP also provides eligible individuals with access to a statewide network of medical subspecialty services. Other benefits currently under negotiation include discounted lab and x-ray services, and pharmaceuticals.


CHAP patients' eligibility are identified by the Delaware Division of Social Services and/or a "Care Coordinators" located at one of twelve locations throughout the state. Care Coordinators can assist individuals in applying to insurance programs, and dependent upon financial eligibility provide linkage for clients to physician resources that comprise the CHAP provider network. The CHAP provider network consists of federally qualified health centers in each of Delaware's counties and the Voluntary Initiative Program Phase II (a formal network of private practice physicians orchestrated by the Medical Society of Delaware).


The goals of CHAP are to link uninsured individuals to insurance resources where possible, and medical health homes, and to improve individual health status, and encourage more appropriate, less costly, use of health resources.


The VIPII is an integral component of CHAP and one of the primary sources of medical homes to which to link CHAP enrollees. The VIPII is a formal network that recognizes the wishes of physicians as patients are assigned to their practices. The daily operation of the VIPII is carried out by a registered nurse who serves as a liaison between patient and physician office staff for initial appointment scheduling. The VIPII nurse coordinator is also the single point of contact for physician's staff to request medical subspecialty services for CHAP patients.
